New employees - please complete the "new employee" form and e-mail to the people
listed on the form. (You can use the pdf form OR the word form.) You will also need a W4 form (link below). You are able to complete the W4 form on-line, print a copy, sign and send to the ESC office. (We do not need page two of the W-4.) Paychecks will not be sent until these forms have been completed.
